Step 2: Student is responsible for requesting documentation from current or previous medical/mental health providers and providing the information to Disability Services for review. Refer to summary of acceptable documentation.
Step 3: Once the documentation is reviewed, the student will be contacted by the Disability Services Coordinator to discuss appropriate accommodations and, if approved, to develop the Accommodation Plan.
Step 4: The Accommodation Plan is sent to the student for review and signature. Step 5: Once the signed copy has been received, the Disability Services Coordinator emails the Accommodation Plan to the student’s instructors and a copy is emailed to the student. Step 6: The student is responsible for contacting the Disability Services office at least 3 weeks prior to the beginning of each semester to update the Accommodation Plan.
